The main reason why this fax form fails is because people add extra zeros. Remember, it's the country code followed by the area code followed by the phone number. No spaces and no extra zeros. The only reason you would put a zero in front of the number is if the zero is part of the number. if unsure then look up the country code and area code of the place you are sending the fax to.
eg. To send fax to Athens Greece you dial the country code 30 then the area code 201 then the phone number. EX: 302016234462
